A pediatrician and a nurse from StayWell Health Center will be available during the school day at three Waterbury middle schools to provide sports physical examinations for the Spring sports season. StayWell’s providers are scheduled to visit the high schools on the following days: March 20 at Westside Middle School, April 3 at North End Middle School and April 4 at Wallace Middle School.
Students interested in trying out for a Spring sport and it has been more than a year since their last physical examination are strongly encouraged to pre-register for an appointment. Permission slips and the required health forms are available in the Main Office and the School Nurses Office at each middle school.
StayWell accepts the HUSKY/Title 19 plan, Medicaid, and the Charter Oak plans. Several of the major private medical insurance plans are also accepted for medical services. Uninsured students, who are not eligible for the HUSKY/Medicaid plan, may be eligible to receive a sports physical examination at no cost.
